Concerned about a child or adult? Use the following guide to help you report your concern.

Under no circumstances should a volunteer or worker carry out their own investigation into an allegation or suspicion of abuse.

If a child or adult is at risk of immediate harm, please contact the police immediately on 999 and report your concern to them BEFORE following the instructions below.

Where it is deemed there is no immediate risk of harm to an individual, the person in receipt of allegations or suspicions of abuse and/or concerns should report this as soon as possible (not later than 24hrs after) by following the instructions below.

Instructions for reporting a concern:

Document the details using the Cause for Concern report below; this helps you to gather relevant information accurately, prior to talking to others about it. Along with details of your concern, you will need for the subject of concern:
* name
* address
* date of birth (for a child) age (for an adult)
Once you access the form you will not be able to save and return to it, so gather the information quickly before starting.
NB: If you don’t have this information, you can still complete the form, but the more information we have the better we can process the concern.

Our Designated Safeguarding Leads are Wendy Williamson and Esther Cotton.

They are nominated by the Leadership to act on behalf of Life Church Lichfield in dealing with the allegation or suspicion of neglect or abuse, including referring the matter on to the statutory authorities. They can be contacted via e-mail, safeguarding@lifechurchlichfield.com.
